This form needs to be filled out before your first visit to our office. It is a brief one page questionnaire about your health history. The information is to assist us in providing the best care possible. Please answer all questions truthfully and completely. Dr. Bridget Doyle grew up in Tulsa, Oklahoma. She received her Bachelor's degree from the University of Oklahoma, and her Doctorate from the University of Oklahoma College of Dentistry in 2005. Dr. Doyle has gained experience working in various practices from Colorado to Virginia, and returned home to Oklahoma in 2010. She takes pride in treating the whole family, and is extremely passionate about being able to help small children in the hospital setting.
Dr. Doyle lives in Tulsa with her Husband, Mark, and they just welcomed their son in March.

The town of Sapulpa was established in 1898. Early railroads build a spur to the area in the 1886, which was know as Sapulpa Station. The town was named after Chief Sapulpa, who was a full blood Creek Indian and the first settler in the area. Sapulpa produced walnuts in the early years. Later it became a source for brick, clay products, and glass. Sapulpa eventually earned the title of "The Crystal City of the Southwest."
Sapulpa's rich heritage can still be seen today in the downtown area around Main Street. With a growing community and a special small town charm, we are honored to be located in Sapulpa.
